Set Default Intake Forms

  Permissions: You must have Organization Administration Permissions.

When you send a Client Portal Invitation you can select which Intake Forms you would like to include. You may also set Default Intake Forms to be selected for each invitation.

Select Default Intake Forms

1. Click Organization in the primary navigation bar.
2. Click Client Portal from side menu.
3. Click Intake Forms tab.
4. Locate desired added Form and click YES to included it in the default Intake Form packet Clients complete in the Client Portal. 
5. Click Save.
   
   
   
6. Scroll down to the Manage Clients section and select the checkbox for a client who does not have a Client Portal invite.
7. Click Invite to Client Portal.
   
   
   
   
8. Select forms you would like to send on invites and make edits to the message if needed.
9. Click Save default message and intake form selections.
10. Click Preview Invitation to see the invitation message and included forms.
    
    
    
    
11. Click Invite to Client Portal to confirm the invitation settings.
