Permissions: Can invite clients to the client portal and manage intake forms must be selected in Default Permissions on a per staff basis to create and send Intake forms for staff with Default, Billing and Therapist permissions.
Administration and Organization Administration permissions include Intake forms.
One signature is collected for intake forms sent as a group. Under the signature it clearly states that the signature will be applied to ALL information contained in the intake forms.
If you require a signature for individual forms they must be sent individually.
If you desire Client acknowledgement on each individual Form in an group of default intake forms, you can edit each Form to include a mandatory Client acknowledgement such as Client Initials and Date using Short Answer field.
