If you created any Dynamic Forms that you would like to use as Intake Forms in the Client Portal, you will need to copy it to the Client Portal. Forms are saved as PDFs on the Client's General Documents tab.

- Click Dynamic Forms in the primary navigation bar.
- Locate desired Dynamic Form to add.
- Click Copy to Intake Forms in the Actions menu.
- This Form will now be available to Send via Client Portal.
