Permissions: You must have Organization Administration Permissions.
There are three standard forms we provide: Standard Intake Questionnaire, Informed Consent for Psychotherapy, and Notice of Privacy Practices. These will always be available to send as Intake Forms through the Client Portal.
If you created any Dynamic Forms that you would like to use in the Client Portal, you will need to Copy it to the Client Portal. Completed Forms as saved as PDFs on the Client's General Documents tab.
Copy Form to Client Portal
- Click Dynamic Forms in the primary navigation bar.
- Locate desired Dynamic Form to add and click Copy to Client Portal.
- This Form will now be available to select/send as part of the Client's Intake Paperwork.
